As orthopaedic surgeons, we think about bones, cartilage, ligaments, tendons, alignment and biomechanics. We repair fractures, reconstruct damaged joints, correct mechanical problems and, when an articulation can no longer perform its function, sometimes replace it.

That way of thinking remains essential. Anatomy matters. Biomechanics matters. Structural damage matters. I still believe strongly in all of it.

But as my career progressed, I became increasingly interested in a different question: why does the structure fail in the first place, and why does the same apparent structural problem behave so differently from one patient to another?

My interest in this did not begin with weight-loss medication, GLP-1 receptor agonists or longevity medicine. In fact, it started many years earlier, through studying skeletal muscle, sarcopenia, regenerative medicine and osteoarthritis.

The deeper I went into the biology of musculoskeletal disease, the harder it became to see the knee, the muscle or the bone as isolated structures. Gradually, the boundary between “orthopaedic disease” and the biology of the person carrying that disease became much less clear.

That journey is ultimately what brought me to metabolic health.

It started with muscle

One of my earliest major academic interests was sarcopenia. I wrote about skeletal muscle loss, strength, ageing and the molecular pathways controlling muscle synthesis and degradation long before the current conversation around metabolic optimisation became fashionable.

Studying skeletal muscle profoundly influenced how I began to think about health.

Muscle is obviously fundamental for movement, balance and strength, but it is much more than a mechanical engine attached to the skeleton. It is one of the body’s major metabolic organs. It plays an important role in glucose disposal and insulin sensitivity, communicates with other tissues through multiple signalling pathways, and is closely interconnected with inflammation, hormonal physiology and ageing.

In my earlier work on sarcopenia I reviewed, among other mechanisms, how inflammatory cytokines can activate NF-κB, interfere with insulin and IGF-1–Akt signalling and favour pathways involved in muscle catabolism.

This creates a biological cycle that is very relevant clinically. When someone becomes weaker, movement becomes more difficult. They move less, lose further strength and physical capacity, and their metabolic health can deteriorate. Reduced muscle quality and inactivity can worsen glucose disposal. Adiposity may increase. Insulin resistance and inflammatory signalling can increase further.

Something that begins as a problem of physical function therefore starts interacting with metabolism.

Years later, I would recognise many of these same relationships when studying osteoarthritis.

The knee taught me to look beyond the knee

For decades, osteoarthritis was described largely as “wear and tear”. The explanation was intuitively attractive: we get older, the cartilage wears down, the joint becomes painful and eventually, in severe cases, we replace it.

The problem is that biology is not quite that simple.

Today we understand osteoarthritis much better as an active disease of the whole joint organ. Cartilage is important, but it is only one component. Subchondral bone, synovial tissue, menisci, ligaments, the infrapatellar fat pad, vascular structures, nervous tissue and the musculature surrounding the joint can all participate in the disease.

My own doctoral work has explored osteoarthritis from this perspective: not as an inevitable passive consequence of ageing, but as a multifactorial disease in which mechanical loading interacts with cellular ageing, inflammatory pathways, metabolic factors, genetics and the wider biological environment of the patient.

Chronic low-grade inflammation is particularly interesting because it does not affect only one part of the articulation. In osteoarthritis it can influence chondrocyte behaviour, extracellular-matrix turnover, synovial activation, subchondral bone remodelling, angiogenesis, neoinnervation and ultimately pain. My doctoral work describes precisely this whole-joint interaction.

Some years earlier, this had already become very tangible to me through laboratory research. In a study we published using human tissue from patients with advanced knee osteoarthritis, we examined cartilage, synovium and Hoffa’s fat pad and found co-expression of FKBP51 with inflammatory markers including TNF-α and IL-6 in different tissues of the osteoarthritic joint.

For me, this was part of a much broader conceptual change.

If an osteoarthritic knee is an organ affected by mechanical, inflammatory and biological processes, then it becomes reasonable to ask what is creating or amplifying those processes.

And one of the answers is metabolism.

Obesity is more than additional weight on a knee

The relationship between obesity and knee osteoarthritis provides perhaps the clearest example.

There is obviously a mechanical component. Increased body mass increases the forces transmitted across a weight-bearing joint, and during walking and other activities those forces can be several times body weight.

But adipose tissue is not an inert storage compartment.

It is an endocrine and immunometabolic organ.

In the setting of dysfunctional visceral adiposity, we often see insulin resistance, altered adipokine signalling, hyperinsulinaemia, oxidative stress and persistent low-grade systemic inflammation. Cytokines such as TNF-α and IL-6 can be elevated and can influence the joint environment as well as other organs.

This is why simply saying that obesity damages the knee because “there are more kilos on it” is incomplete.

The knee experiences the mechanical weight of the patient, but it is also exposed to the metabolic, hormonal and inflammatory environment of that patient.

The same concept becomes relevant with ageing. Cellular senescence, mitochondrial dysfunction and the senescence-associated secretory phenotype can create an increasingly inflammatory and catabolic environment. It is also relevant in women around menopause, when changes in sex hormones frequently coincide with changes in visceral fat, insulin sensitivity, muscle and bone.

None of this means that every osteoarthritis is caused by metabolic dysfunction. Post-traumatic osteoarthritis, malalignment, genetic susceptibility and other mechanical factors clearly exist and are extremely important.

But the common patient we see in clinical practice is often much more complex than an isolated injured cartilage surface.

Ageing, obesity, metabolic syndrome, declining muscle function and, in many women, menopausal hormonal change can overlap.

The disease in the X-ray may look similar. The biological environment in which it developed may be very different.

That distinction increasingly matters to me.

At some point, the question also became personal

For years my interest in metabolism had been mainly scientific and clinical.

Then a routine health assessment turned part of that curiosity towards myself.

Despite being physically active and having spent most of my adult life interested in exercise and physiology, my lipid and metabolic profile was not as optimal as I wanted it to be.

There was no dramatic diagnosis. I was not suddenly “ill”. But that was exactly what made the experience interesting.

It changed the question from:

Do I have a disease that needs treatment?

to:

What could I improve now to reduce the probability of developing disease later?

I began looking more closely at my own body composition, visceral adiposity, insulin sensitivity, cardiovascular risk, lipid profile, sleep, nutrition, exercise, muscle mass and hormonal physiology.

That personal process reinforced something I had increasingly realised professionally: waiting until a biological process crosses an arbitrary threshold and receives the name of a disease is not always the most intelligent moment to begin paying attention to it.

My subsequent training in Integrative Medicine allowed me to study many of these relationships within a broader framework. It did not replace conventional medicine. Quite the opposite: it made me more interested in connecting evidence from different areas of conventional physiology and medicine that we often separate because they belong to different specialties.

The body, of course, does not care whether we classify something as orthopaedics, endocrinology, cardiology or nutrition.

Insulin resistance does not stop influencing physiology because the patient has entered an orthopaedic consulting room.

Neither does menopause.

Neither does visceral adiposity.

Neither does muscle loss.

This was the background against which I started following the development of modern GLP-1 therapies much more closely.

Why the GLP-1 story became so interesting

GLP-1 receptor agonists did not begin as longevity drugs or obesity drugs. They emerged primarily from diabetes medicine.

Then came the extraordinary weight-loss data, and drugs such as semaglutide became known to the general public largely because of their effects on body weight.

More recently, tirzepatide added GIP receptor agonism to GLP-1, and retatrutide — still investigational — goes a step further by combining agonism of GIP, GLP-1 and glucagon receptors.

But concentrating only on the weight loss risks missing what may be the most interesting part of this story.

If these drugs simply made people thinner, they would still be highly important treatments for obesity.

But the clinical evidence is increasingly showing effects across apparently different diseases and organ systems.

Consider cardiovascular disease. In the SELECT trial, 17,604 people with overweight or obesity and established cardiovascular disease, but without diabetes, were randomised to semaglutide or placebo. Major cardiovascular events — cardiovascular death, non-fatal myocardial infarction or non-fatal stroke — occurred significantly less frequently with semaglutide, corresponding to approximately a 20% relative risk reduction.

That moved the discussion well beyond cosmetic weight loss.

Then came the kidney. In the FLOW trial, involving patients with type 2 diabetes and chronic kidney disease, semaglutide reduced the risk of the major kidney/cardiovascular composite outcome by 24%, while several important secondary outcomes also favoured treatment.

Other trials have expanded the conversation into metabolic liver disease, heart failure and obstructive sleep apnoea.

Different organs. Different specialties. Different diagnoses.

But again and again, the diseases overlap with obesity, insulin resistance and metabolic dysfunction.

And then these drugs reached my own field.

GLP-1 and osteoarthritis: this is where it became particularly interesting for me

The STEP 9 trial was one of the studies that most caught my attention.

It enrolled 407 people with obesity and symptomatic knee osteoarthritis. After 68 weeks, participants receiving semaglutide 2.4 mg had lost an average of 13.7% of their body weight compared with 3.2% in the placebo group.

That result was expected to some extent.

What was more interesting from an orthopaedic perspective was what happened to the knee.

The WOMAC pain score decreased by 41.7 points with semaglutide compared with 27.5 points with placebo, and physical function improved significantly more in the semaglutide group.

A medication developed from metabolic medicine was producing a meaningful clinical effect in symptomatic knee osteoarthritis.

Then came retatrutide.

Retatrutide is an investigational triple GIP/GLP-1/glucagon receptor agonist, rather than a conventional GLP-1 receptor agonist. The phase 3 TRIUMPH-4 trial was particularly interesting because it enrolled adults with overweight or obesity and knee osteoarthritis without diabetes.

At 68 weeks, participants receiving 12 mg lost an average of 28.7% of their body weight. WOMAC pain fell substantially, by around 4.4–4.5 points in the two active high-dose groups. In post-hoc percentage analyses that represented reductions of roughly 74–76%, and approximately 12–14% of treated patients were reported to be completely free from knee pain at 68 weeks, compared with 4.2% with placebo. These are company-reported phase 3 topline results and retatrutide remains investigational, so they need to be interpreted accordingly until the complete peer-reviewed dataset is available.

Nevertheless, from an orthopaedic perspective those are difficult numbers to ignore.

And they immediately raise the most important question:

Is all of this simply because the patient weighs less?

Weight matters — but it may not be the entire explanation

Clearly, substantial weight loss changes knee biomechanics.

If someone loses 15, 20 or almost 30% of their body weight, mechanical loading across the joint decreases. That alone can improve symptoms and physical function.

There is no reason to minimise that effect.

But metabolic improvement accompanies the mechanical unloading. Visceral adiposity falls. Insulin sensitivity can improve. Blood pressure, lipids and systemic inflammatory markers can change.

The patient is not simply carrying less mass; the internal metabolic environment may also be substantially different.

More provocatively, there is now experimental evidence suggesting that at least some GLP-1 effects on osteoarthritis biology might not be entirely dependent on weight loss.

A 2026 study published in Cell Metabolism examined semaglutide in obesity-associated osteoarthritis and used controlled feeding experiments specifically designed to separate some of the appetite and weight-loss effects of the medication. The investigators identified signalling through a GLP-1R–AMPK–PFKFB3 pathway and changes in chondrocyte metabolism, while observing improvements in experimental osteoarthritis pathology.

This is biologically fascinating, particularly because a recent review in the same journal has highlighted weight-loss-independent actions of GLP-1 medicines through neural, immune and inter-organ pathways.

But this is also where we need scientific discipline.

These findings do not mean that semaglutide has been proven to regenerate human cartilage.

We cannot currently call GLP-1 receptor agonists established disease-modifying osteoarthritis drugs.

And we certainly cannot extrapolate these findings to prescribing GLP-1 medication specifically for osteoarthritis in metabolically healthy, normal-weight patients.

At present, the strongest clinical evidence comes from people with overweight or obesity.

The weight-independent mechanisms are an exciting scientific hypothesis supported primarily by experimental biology, not a finished clinical conclusion.

That distinction matters.

Perhaps we have been asking too narrow a question

For decades, much of osteoarthritis research has understandably asked whether we can develop something that protects or regenerates cartilage.

I have worked in regenerative medicine myself and know how difficult this objective is.

Articular cartilage is an extraordinary tissue but it has very limited intrinsic capacity for repair. It is hypocellular, avascular and exists in an extremely demanding biomechanical environment. Our earlier work on mesenchymal stem cells and cartilage regeneration discussed precisely the biological challenges involved in trying to restore damaged cartilage.

Perhaps, therefore, part of the future does not lie exclusively in finding something that acts directly on cartilage.

Perhaps we also need to ask:

What biological environment are we asking that cartilage to survive in?

Imagine a patient with visceral obesity, insulin resistance, poor muscle quality, chronic low-grade inflammation, sleep apnoea and painful knee osteoarthritis.

We can describe that person as having six different medical problems.

And technically they do.

But biologically those problems are not entirely independent.

The metabolic dysfunction contributes to the adiposity and inflammation. The knee pain reduces activity. Reduced activity worsens muscle function and insulin sensitivity. Poor sleep worsens appetite control and metabolic health. Increasing weakness makes exercise even more difficult.

The feedback loop begins to reinforce itself.

Now imagine beginning to reverse that direction.

Fat mass decreases.

Insulin sensitivity improves.

The knee hurts less.

The patient moves more.

Strength training becomes possible again.

Muscle function improves.

Cardiovascular fitness improves.

Sleep may improve.

The metabolic environment changes further.

At that point, it becomes increasingly artificial to ask which improvement belongs to orthopaedics and which belongs to metabolic medicine.

They are happening in the same human being.

The muscle remains essential

There is, however, an important counterbalance to the excitement surrounding powerful weight-loss therapies.

When somebody loses a large amount of weight, not everything lost is adipose tissue. Some lean mass can also be lost.

This is particularly relevant to me because of my background in sarcopenia and musculoskeletal medicine.

A lighter patient is not automatically a healthier or more functional patient.

A patient with osteoarthritis needs muscle, particularly lower-limb strength. Muscle contributes to joint stability, shock absorption and functional capacity, but also to metabolic health and bone loading.

As we age, maintaining muscle becomes even more important because loss of strength is strongly associated with frailty, falls, disability and loss of independence.

For that reason, I do not believe the intelligent use of modern obesity pharmacotherapy is simply:

take an injection and eat as little as possible.

The more rational model is to combine appropriate metabolic treatment, when medically indicated, with progressive resistance training, adequate protein, cardiovascular activity, good nutrition, sleep and attention to bone health.

The objective should not be to make the scale display the smallest possible number.

It should be to reduce dysfunctional adiposity while preserving as much muscle, strength and physical capacity as possible.

That is a very different definition of successful weight loss.

And this is where the conversation begins to touch healthy ageing

The word “longevity” is currently used very freely, sometimes too freely.

I do not think we should call GLP-1 receptor agonists human longevity drugs at this point. We do not have evidence that semaglutide, tirzepatide or any other drug in this family extends human lifespan.

But there is another way of looking at the question.

If a therapy reduces major cardiovascular events, slows progression of kidney disease in appropriate populations, improves severe obesity and metabolic dysfunction, improves sleep apnoea and helps some patients with obesity and knee osteoarthritis regain mobility, then it is affecting several of the diseases that progressively remove healthy years from later life.

That is already highly relevant to healthspan.

And very recently the longevity question has become even more intriguing experimentally.

In September 2026, researchers reported that starting semaglutide late in life in 20-month-old female mice improved several physiological and molecular features associated with ageing and extended median lifespan.

It is an exciting result.

It is also a mouse study.

A mouse living longer after semaglutide does not mean that a human being will live longer by taking semaglutide, and we should resist turning an interesting geroscience finding into a clinical claim before the evidence exists.

But scientifically, it adds another piece to an increasingly interesting puzzle.

What happens when we profoundly improve energy metabolism, insulin signalling, adiposity, inflammatory biology and inter-organ communication?

Could some of what we currently call “age-related disease” be influenced by intervening earlier on those systems?

I think that is a question worth asking.

Why I am starting to talk more about metabolism, hormones and healthy ageing

For patients who have known me primarily as an orthopaedic surgeon, some of the subjects I will increasingly discuss here may initially seem like a change of direction.

In reality, I see them as a continuation of the same journey.

I started by studying and treating structural disease.

Sarcopenia led me into skeletal muscle physiology and the relationship between muscle, inflammation and metabolism.

Regenerative medicine made me appreciate how profoundly the biological environment determines tissue repair.

Osteoarthritis taught me that a damaged joint is not simply damaged cartilage, but a whole organ interacting with the biology of the patient.

Research into chronic low-grade inflammation made the connection even clearer.

My own metabolic health made the subject personal.

Further training in Integrative Medicine gave me a broader framework for connecting metabolism, body composition, hormonal physiology, exercise, nutrition, sleep and long-term disease risk.

And now the development of GLP-1 receptor agonists and newer incretin therapies is forcing medicine to reconsider some of the artificial boundaries we have placed between apparently different diseases.

I remain an orthopaedic surgeon. I still believe deeply in biomechanics, structural diagnosis, joint preservation and surgery when surgery is indicated.

But I also increasingly believe that treating structure without considering biology gives us only part of the picture.

For some patients, the future of osteoarthritis treatment may involve surgery.

For others it may involve rehabilitation, strength and weight reduction.

And for a particular metabolic phenotype, treatment may increasingly include correcting the systemic biological environment that is helping drive the disease.

The same principle may apply much more widely than osteoarthritis.

This is one of the reasons why, alongside my traditional musculoskeletal work, I will increasingly be writing here about metabolic health, body composition, hormonal physiology, GLP-1 therapies and healthy ageing.

Not because I have moved away from orthopaedics.

Quite the opposite.

Orthopaedics is one of the things that taught me to look beyond the joint.

And perhaps the most important question is no longer simply:

What disease does this patient have?

but also:

What biology is driving it, what can still be changed, and how early should we begin?

That is a conversation I intend to explore much more over the coming months.

Medical note

The information in this article is intended for education and discussion and does not constitute individual medical advice.
